What if you could improve your workout performance without actually
swallowing anything? A simple mouth rinse containing carbohydrates or caffeine
has attracted attention in sports science for its potential to influence the
brain, strength, muscular endurance, and focus.
But does it really work—or is it just another fitness trend? Research
suggests the answer is more interesting than a simple yes or no. Let’s look at
what the science says about carbohydrate and caffeine mouth rinsing and whether
it has a place in serious training.
Can you improve exercise performance without
actually swallowing a carbohydrate or caffeine drink?
It sounds unusual, but sports scientists have
been studying a technique called mouth rinsing—swishing a carbohydrate,
caffeine, or combined solution around the mouth for several seconds and then
spitting it out.
The idea is simple: certain substances may
interact with receptors in the mouth and influence the nervous system without
requiring the athlete to consume the full dose.
But does it actually make you stronger?
Can it help you perform more repetitions?
And could it improve concentration during
training?
The research provides a much more interesting
answer than a simple yes or no.
What Is
Mouth Rinsing?
A mouth rinse is a small amount of liquid that
is swished around the mouth for a short period and then expectorated rather
than swallowed.
Sports researchers have investigated solutions
containing:
- Carbohydrate
- Caffeine
- Carbohydrate + caffeine
- Menthol
The proposed mechanism is partly related to
receptors in the mouth that can send signals to the brain. Researchers have
suggested that these signals may influence reward, motivation, effort
perception and motor control.
This is different from drinking a carbohydrate
or caffeine beverage because the objective is to stimulate oral receptors while
minimising ingestion.
What Did
the Original Strength Study Find?
A 2021 randomised crossover study examined 27
resistance-trained participants—13 men and 14 women.
Participants rinsed their mouths with:
- 6% carbohydrate;
- 2% caffeine;
- carbohydrate + caffeine; or
- water placebo.
They then completed tests involving squat and
bench-press 1RM, muscular endurance and cognitive performance.
The researchers found no significant
improvement in squat or bench-press 1RM strength.
However, the combined carbohydrate-caffeine
rinse improved squat endurance during the first set in both men and women.
Cognitive performance also improved compared with placebo.
That distinction is important:
Maximum strength: no significant improvement
Lower-body muscular endurance: improvement in this study
Cognitive performance: improvement in this study
So mouth rinsing should not be described as a
shortcut to becoming stronger.
What Does
the Larger Evidence Say?
A systematic review and meta-analysis of
carbohydrate mouth rinsing found no significant effect on maximal muscle
strength, but it did find a positive effect on muscular endurance.
Across the included trials, the estimated
improvement was about 1.24 repetitions, although results varied between
studies.
This suggests that carbohydrate mouth rinsing
may be more relevant to repeated-effort performance than to maximal
strength.
In other words, it may help you squeeze out
additional work under certain conditions, but it does not appear to
meaningfully increase your one-repetition maximum.
What About
Caffeine Mouth Rinsing?
Caffeine is particularly interesting because
normal caffeine consumption is already well known as an exercise-performance
strategy.
Mouth rinsing attempts to obtain some of
caffeine's potential effects without consuming the full amount.
Earlier systematic reviews found mixed
results. One review identified 11 randomised crossover studies; only three
reported significant improvements in physical performance, while most found no
clear improvement or only suggestive effects.
More recent evidence is somewhat more
encouraging—but still does not justify treating caffeine mouth rinsing as a
guaranteed performance enhancer.
A 2026 meta-analysis covering 31 studies
and 167 effect estimates found small improvements in exercise performance
overall. Benefits appeared more consistent for aerobic endurance, while
cognitive effects were inconsistent and the certainty of evidence for cognition
was low.
What
Happens When Carbohydrate and Caffeine Are Combined?
This is where the research becomes
particularly interesting.
A 2024 systematic review examined combined
carbohydrate-caffeine mouth rinsing and included nine randomised crossover
studies.
Only one study showed a statistically
significant improvement in lower-body muscular endurance. Several other
physical-performance measures—including upper-body strength and
endurance—generally did not show significant benefits.
However, both studies that examined cognitive
performance reported significant improvements with the combined rinse compared
with placebo.
The researchers concluded that a clear
synergistic effect on physical performance had not been established,
while preliminary evidence suggested possible cognitive benefits.
Why Might
Mouth Rinsing Work?
Scientists have proposed several possible
mechanisms.
1. Oral
receptors
Carbohydrate and caffeine may interact with
receptors in the mouth.
Those receptors can send signals to the brain
without requiring the substance to be swallowed.
2. Reward
and motivation
Carbohydrate mouth rinsing has been proposed
to activate brain regions involved in reward and motivation.
3.
Perception of effort
Some studies suggest that mouth rinsing may
influence how difficult exercise feels.
If perceived effort changes, an athlete may
potentially maintain performance for longer.
4. Central
nervous system stimulation
Caffeine may influence neural processes
associated with alertness and arousal.
This may help explain why caffeine mouth
rinsing has attracted interest for both exercise and cognitive performance.
However, these mechanisms remain an area of
active research.
Does It
Build Muscle?
Not directly.
Mouth rinsing is not a muscle-building
supplement in the traditional sense.
It does not provide protein, essential amino
acids or meaningful calories when the solution is spat out.
If it improves training performance by
allowing an athlete to complete more quality work, there could theoretically be
an indirect benefit to training adaptations—but this has not been established
as a direct muscle-growth effect.
For bodybuilding, fundamentals remain much
more important:
Progressive resistance training + adequate
protein + sufficient calories + sleep + recovery
Mouth rinsing, at most, would be an optional
performance strategy.
Could It
Help During a Hard Workout?
Potentially—but the evidence depends heavily
on the type of exercise.
The newer 2026 network meta-analysis found
that carbohydrate and caffeine mouth rinses could improve certain endurance
outcomes compared with placebo, including time to exhaustion and work completed
during fixed-time exercise. However, the authors emphasised that the relative
differences between carbohydrate, caffeine and menthol were not statistically
significant.
The evidence is therefore more promising for endurance
and fatigue-related performance than for maximal strength.
What About
Cognitive Performance?
This may be one of the more interesting
applications.
The 2021 resistance-training study found
improved cognitive performance after combined carbohydrate-caffeine mouth
rinsing.
The 2024 systematic review also found
preliminary evidence of cognitive benefits from combined carbohydrate-caffeine
mouth rinsing.
However, the newer 2026 caffeine-only
meta-analysis found that cognitive effects were inconsistent overall, with low
certainty of evidence.
So the scientifically responsible conclusion
is:
There is a signal worth investigating, but the
cognitive benefits are not yet established as a reliable effect.
High-Load
Bodybuilding vs Endurance Exercise
This distinction matters.
A mouth rinse might behave differently during:
Heavy 1–5 repetition strength training
than during:
Longer-duration or repeated-effort exercise.
Current evidence does not support claiming
that carbohydrate or caffeine mouth rinsing substantially increases maximal
strength.
The evidence is more favourable for certain
endurance and muscular-endurance outcomes.
Is Mouth
Rinsing Better Than Drinking Caffeine?
Not necessarily.
Traditional caffeine consumption has
substantially more evidence behind it as an exercise-performance strategy.
Mouth rinsing is interesting because it may
provide a way to stimulate oral and neural pathways without consuming as much
caffeine.
But it should be viewed as a specialised
experimental strategy, not a replacement for established nutrition and
performance practices.
Important
Limitations
The studies in this area have several
limitations.
Many trials involve relatively small numbers
of participants.
Exercise protocols differ substantially
between studies.
Caffeine doses, carbohydrate concentrations
and rinse durations also vary.
Participants may differ in their habitual
caffeine consumption and nutritional status.
The 2026 caffeine meta-analysis specifically
identified heterogeneity and methodological concerns across the available
research.
These limitations make it difficult to give
athletes one universal mouth-rinsing protocol.
The Bottom
Line
Carbohydrate and caffeine mouth rinsing is an
intriguing area of sports nutrition.
The evidence suggests that:
Maximum strength:
Little evidence of a meaningful benefit.
Muscular endurance:
Carbohydrate mouth rinsing may provide a small benefit in some circumstances.
Endurance exercise:
Caffeine and carbohydrate mouth rinsing may improve certain performance
measures.
Cognitive performance:
